Enrollment

Pupils must be five years of age on or before September 1 to be admitted to kindergarten the preceding September. 
Physical examinations are required prior to the entrance into kindergarten or first grade and upon entrance into the fifth and ninth grades. 
These may be dated within one calendar year of enrollment in kindergarten or first grade and within one calendar year of enrollment of fifth year.  Pupils who transfer into our district must present evidence of such examination in accordance with the laws of the State of Illinois.  Additional health examinations may be required when deemed necessary by the school authorities.  In addition to the physical examination every child entering kindergarten or first grade and every transfer student who has not previously complied with the law must be immunized against measles, tetanus, diphtheria, poliomyelitis, and pertussis.
When these students appear for enrollment with parents, the necessary information can be obtained and the proper forms completed in the office of the principal.  When a new student appears for enrollment without a parent, the student will be given a blank enrollment sheet with a note attached requesting the parent to complete the enrollment sheet and give any other necessary information.  Please include numbers where you can be reached during the day if not at home.
 No student will be considered as enrolled in school until the parent or legal guardian has signed the enrollment sheet stating his/her address and whether he/she is a resident of our district.  The enrollment sheet is to contain the legal name of the child, as should all other school records.  Items necessary for enrollment are these:
 
1.         A birth certificate for students entering kindergarten or first grade, and any transfer student;
2.         A completed enrollment sheet signed by the parent or legal guardian;
3.         Evidence of required immunizations;
4.         A report card or other official record of work done in previous schools.
 

Early Child Find and Identification

Pre-K and Early Childhood at Risk programs are available to students, ages 3-5 years, provided they meet eligibility requirements established by law.  Screening for these programs and for kindergarten students are scheduled and conducted each fall and then again in the spring.

School Assignment

Students enrolled in kindergarten through fourth grade will attend Farmingdale Elementary School.  Services offered beyond the core curriculum, include: Special Reading, Gifted Enrichment, Speech Therapy, Supportive Resource Services, Music, Art, Physical Education, and Technology Resources.

Bus Assignment

 The director of transportation assigns pupils to a particular bus.  Pupils who wish to ride buses to which they are not assigned in order to visit friends may do so only under three conditions:  (1) there must be sufficient room on the bus; (2) a request must come in writing from the parent or guardian; and (3) the request must be approved by the building principal.
