Types of Student Records
It is the responsibility of the building principal to see that two types of official records are kept for each student, the permanent record and the temporary record. No other official records are to be kept. Official records are defined to be those records that are accessible to at least two staff members. A staff member for personal use may keep unofficial records. No restrictions are being placed on unofficial records by this policy. The permanent record is to contain only: a. Basic identifying information, including the student's name, the name of the parents, their addresses, and the birth place, birth date, and gender of the student; b. Academic transcript, including grades, class rank, graduation date, grade level achieved, and scores on college entrance examinations; c. Attendance records; d. Accident reports and health records; e. Record of release of permanent record information; f. Honors and awards received at the high school level (to be included upon student request); g. Information concerning participation in school sponsored activities or offices held in school-sponsored organizations at the high school level (to be included upon student request). The temporary record may include only the following: a. Family background information; b. Intelligence test scores, group and individual; c. Aptitude test scores; d. Reports of psychological evaluations including information on intelligence, personality, and academic information obtained through test administration, observation, or interviews; e. Elementary and secondary achievement level test results; f. Teacher anecdotal records; g. Disciplinary information; h. Special education files including the report of multi-disciplinary staffing on which placement or non-placement was based, and all records and tape recordings relating to special education placement hearings and appeals; i. Any verified reports of information from non-educational persons, agencies, or organizations; j. Other verified information of clear relevance to the education of the student; and, k. Record of release of temporary record. No person may condition the granting or withholding of any right, privilege or benefit or make as a condition of employment, credit or insurance the securing by any individual of any information from a student's temporary record which such individual may obtain through the exercise of any right secured under the Illinois School Students Records Act.
Student Records - Rights of Parents
Parents of students in our district have the right to: 1. Inspect and copy permanent and temporary records; 2. Control access and release of school student records; 3. Request a copy of information released; and, 4. Challenge the content of the school record. The school shall grant access to, or release information from school student records WITHOUT PARENTAL CONSENT OR NOTIFICATION: 1. To an employee or official of the school or school district or to an employee or official of the State Board of Education provided such employee or official has a current demonstrable interest in the student and the records are pertinent to such interest; 2. To any person for the purpose of research, statistical reporting, or planning, provided that: a. Such person has the permission of the State Superintendent of Education; and, b. No student or parent can be identified from the information release. 3. Pursuant to a court order provided that the procedures outlined in Ill. Rev. Stat. 1975, Ch. 122, Article 50-6 (a) (5) are observed. Information may be released by the school principal or the school nurse WITHOUT PARENTAL CONSENT in connection with an emergency to appropriate persons if the knowledge of such information is necessary to protect the health or safety of the student or other persons, provided that the parents are notified as soon as possible of the information released, the date of the release, the person, agency, or organization receiving the information, and the purpose of the release. Any school employee may release such information in the absence of the school nurse and principal if, in the opinion of the employee, the release of such information is vital to the life, safety, or health of some person. The school employee should report his action to the appropriate principal as soon as possible.
